What’s the best way to administer testosterone?
When it comes to testosterone therapy, how it’s delivered matters and not all methods are equally effective or appropriate. For men, injections are generally the most reliable and effective method. To get the most out of therapy, injections must be precisely dosed and administered frequently enough to mirror the body’s natural rhythm. This helps maintain stable hormone levels, reduces peaks and crashes, and delivers better symptom control. We teach patients how to self-administer injections at home, minimizing time spent in the clinic and supporting long-term consistency and adherence in real life. Topical creams and gels are available, but achieving optimal levels in men often requires high doses. Absorption can be inconsistent, and managing treatment becomes more difficult as a result. Generally, we reserve topicals for men who are highly averse to injections. For women, hormone levels are much lower, and topical therapy is quite effective. Specially compounded creams allow for precise, low-dose titration. Commercial injectable options are too concentrated to provide the micro-dosing women require, so injections are rarely appropriate. As for pellets we advise caution. Pellets lack the level of precision that we expect for our patients and are difficult to adjust or remove once inserted. Many patients experience uncomfortable highs and lows, with no ability to fine-tune dosing. Because precision and flexibility are key to safe, effective therapy, we’ve chosen not to offer pellet treatment for men or women.
